Continuous Care for the Challenging Moments

Rainbow Community Care offers continuous care, a specialized level of care that provides intensive nursing support for 8 to 24 hours a day when a hospice patient is experiencing an acute symptomatic crisis at home. Also known as “crisis care”, this level of care is designed to manage severe pain or other distressing symptoms that cannot be controlled with routine hospice care, allowing the patient to remain in their home instead of being moved to an inpatient facility.

What it is

  • A short-term level of care provided during a "crisis" period when symptoms are difficult to manage. 
  • Provided by hospice staff who stay with the patient for extended periods, from 8 to 24 hours a day. 
  • Concentrated on nursing care and skilled interventions to manage severe symptoms like uncontrolled pain, respiratory distress, or agitation. 
  • Often supplemented with hospice aide services to assist with the patient's needs.
  • Sometimes includes a social worker or chaplain who provides emotional and spiritual support to the patient and family. 

When it is appropriate

  • When a patient's symptoms become severe and require more intensive monitoring and intervention than routine home care can provide.
  • When a primary caregiver is no longer able to provide the necessary skilled care.
  • When the goal is to prevent a hospital admission or a move to a general inpatient hospice facility.

How it works

  • Duration: It is not a permanent solution and is provided only as long as necessary to get the crisis under control.
  • Transition: Once the crisis subsides, the patient is typically transitioned back to the more common routine home care.
  • Location: It can be provided in the patient's private residence, an assisted living facility, or a long-term care facility.
