Anyone can refer a person to Rainbow Community Care for services. Please use this page to initiate the conversation.

Call us any time of the day or night at (920) 674-6255  to make a referral. During normal business hours, you will speak with one of our Intake Specialists, either Jodi Badura (pictured above with Molly the Therapy Dog) or Joelle Traynere. After hours, you will talk to one of our Triage Nurses.
