Rainbow Community Care Releases Living Well: Proactive Choices for Aging, Illness, and Loss

New 100+ Page Community Resource Aims to Help Individuals and Families Make Informed Decisions Before a Crisis Occurs

JEFFERSON, WI - Rainbow Community Care is pleased to announce the release of Living Well: Proactive Choices for Aging, Illness, and Loss, a new complimentary community resource designed to help individuals and families throughout rural south-central Wisconsin navigate the challenges and opportunities that accompany aging, serious illness, caregiving, and loss.

The 100+ page publication brings together practical information, local resources, expert insights, and real-life stories from community members who have faced important healthcare and caregiving decisions. The guide was developed to encourage a more proactive approach to planning and decision-making, helping individuals and families better understand their options before a crisis occurs.

"Too often, conversations about aging, serious illness, caregiving, and end-of-life preferences begin during moments of uncertainty and stress," said Carol Brown, RN, President & CEO of Rainbow Community Care. "Our hope is that Living Well empowers people to start those conversations earlier, understand the resources available to them, and make informed decisions that reflect what matters most in their lives."

Topics covered in the publication include:

The publication reflects Rainbow Community Care's belief that living well does not happen by chance. Rather, it is the result of informed, proactive choices made with knowledge, support, and intention.

In addition to being available online, members of Rainbow's Business Development team will be distributing complimentary printed copies throughout the organization's service area, including assisted living facilities, nursing homes, hospitals, clinics, churches, public libraries, and senior citizen centers.
