What Does Hospice at Home Include?

Our in-home hospice services are built around your loved one’s goals and comfort. Care is provided by a team of professionals who visit regularly and remain on call 24/7. Services include:

• Nursing care and case management
  Coordinates all phases of care, as per the physician’s orders. They assess and monitor the patient’s condition and physical status and provide care for pain and symptom management. They are available 24 hours a day, seven days a week.
• Certified nursing assistants (CNAs)
  CNAs help with bathing, grooming, and personal care to preserve dignity and comfort.
• Medical equipment and supplies
  Everything from hospital beds to wound care supplies is delivered to the home.
• Social work support
  Offers education and counseling for both the patient and their family, and can help with financial and legal concerns and referrals to community resources.
• Spiritual care
  Our chaplains offer respectful, inclusive guidance rooted in your loved one’s beliefs and values.
• Volunteer support
  Trained volunteers offer companionship, run errands, or simply sit quietly during moments that matter.

Respite Care When You Need to Recharge

Caring at home can be stressful, even with support. Our Hospice Respite Care service gives short-term relief for you to have time to rest and recover. Your loved one will receive round-the-clock care at our Rainbow Hospice Inpatient Center, ensuring continuity and peace of mind.

Respite care is often covered by Medicare and Medicaid and is available to any eligible family under our hospice program.

Transitioning to Inpatient Hospice Care

Sometimes, symptoms become difficult to manage at home. In those cases, short-term stays at our Inpatient Center can provide advanced symptom control in a peaceful, home-like environment.
